The Liver Health Full Process Management Project of the Community Practice Base Has Been Officially Expanded

At the 8th East China Health Management Forum held in September this year, the "Following Guidelines, Joining Hands with Communities - Empowering Liver Health Full Process Management Project in the Community " was officially launched, aiming to build a four in one "hospital-community-family-individual" health management model with "people-centered, family unit, community scope, and prevention oriented", starting with the completion of the first practical base in Xietu Street Community, The Health Management Center of Zhongshan Hospital Affiliated to Fudan University implements the national graded diagnosis and treatment policy, comprehensively promoting the health management of medical consortium hospitals from hospitals to communities and families.

Recently, at the "Innovation and Integration National Continuing Education Training Class" held at Zhongshan Hospital affiliated to Fudan University, the Liver Health Full Process Management Project was officially expanded for the first time. Besides Xietu Road Street Office, Qingpu Central Hospital, Xuhui Central Hospital, Jinshan Central Hospital, Zhongshan Xiamen Hospital, Wusong Central Hospital, and Minhang Central Hospital, added 4 more grassroots community health service centers: Tangqiao Community Health Service Center in Pudong New Area, Qingcun Town Community Health Service Center in Fengxian District, Sunqiao Community Health Service Center, and Xuhui Xietu Community Health Service Center. This way, it added an important part "community" in the " four phases, one body" - “Hospital-Community-Family-Individual".

On the morning of November 8, Jiang Sunfang, director of the Health Management Center of Zhongshan Hospital affiliated to Fudan University, shared the current situation of community health management in the report. Against the background of severe aging of the population, high incidence rate of chronic diseases, and rising medical insurance costs, community health management is imperative. She pointed out that the current situation of diagnosis and treatment of liver cancer in China is severe - new liver cancer in China accounts for more than 50% of the world, and the five-year survival rate is only 14%, 70-80% of patients are already in the middle and late stages of their initial diagnosis, and one important reason is the lack of early detection and intervention measures and behaviors. Therefore, She hopes to build a Zhongshan-model health management service system, build a health management platform for transforming medical resources into applied community service scenarios, collaborate with medical consortia and community, do a good job in the management, intervention, and screening of chronic liver disease in the community, and form a set of precise and integrated screening model according to guidelines. In this way, hoping to fundamentally reduces the proportion of late stage liver cancer patients and improves the 5-year survival rate.

In achieving early diagnosis, peripheral blood has become a convenient and easy to collect option that contains rich biomarkers and information. Therefore, Director Jiang Sunfang emphasized the active exploration of peripheral blood biomarkers and model algorithms in the early diagnosis of liver disease.
